You can schedule an appointment by calling us at 757-656-3465.

Do I need a referral?

If you wish to use insurance that requires you to see a Primary Care Physician before you can see a specialist, you will need a referral.

If your insurance does not have this requirement or if you do not have insurance, you will not need a referral.  You will also not need a referral if you decide not to use your insurance and to pay for your services yourself.

Please understand that your insurance may not cover your visit if a referral is required and you do not get one.

What should I bring for my appointment?

If you have insurance, please bring your insurance card and your co-pay.  If you are self-pay, please bring your form of payment.

For your first appointment, please bring some form of identification, such as a driver’s license.  You should also bring your medical information because you will be asked to complete a Medical History and a Pain Assessment form and you will need accurate information.

What forms of payment do you accept?

We accept your debit or credit card, cash or a check.  You will make your check payable to the Eastern Shore Pain Management Center.  Payment is due at the time of service.

If I run out of my medication will I be able to get it on the weekend?

If you run out of your medication over the weekend, you will not be able to get a new prescription over the weekend.  For this reason, it is important to be mindful of your prescription and when it will run out so that you can make appropriate arrangements ahead of time.

Will there be anyone on call after hours?

We do not have physicians on call.  If you have an emergency, please go to the emergency room.

If I lose my medication will I be able to get anymore?
